Le Dizzy Horse, un ultra-marathon hivernal, se déroule au Bonneville County Exhibition Hall et au Sandy Downs Park à Idaho Falls, dans l'Idaho. Les participants courent une boucle de 4,167 miles toutes les heures jusqu'à ce qu'il ne reste plus qu'un seul coureur. L'événement propose deux options de participation : « Bareback » avec un soutien minimal ou « Thoroughbred » avec un accès à un poste de secours de luxe. Le terrain est varié et comprend de l'herbe, de la terre et du bitume, avec des conditions neigeuses possibles en décembre. La course se poursuit jusqu'à ce qu'il ne reste plus qu'un seul coureur, le « Last Horse Standing », avec des récompenses pour différentes étapes importantes. Une équipe d'assistance est disponible, mais ne peut pas aider les coureurs sur le parcours.
